Active Temperature Management

Infants and children with acute brain injury benefit from active temperature management as key component of supportive care. Multiple studies have demonstrated sustained euthermia is associated with improved outcomes and thus we recommend maintaining euthermia by a variety of modalities.
 

 

Definition of Euthermia: Core body temperature between 35 º - 38 º C

 

Medications:

  • Acetaminophen 15mg/kg/dose (max 4g/24h) IV or PO/PGT/NG

Equipment:

  • Cooling blanket:

    • Blanketrol III cooling unit with 1 temperature probe adapter cable and 2 sets of connecting hoses.

    • 1-2 appropriate sized Maxi-Therm Lite cooling blanket (adult or pediatric sizes)

  • Temperature Probe (esophageal or rectal)

  • Temperature Regulation on ECMO

    • Temperature is regulated using the ECMO circuit rather than a cooling blanket.

 

Procedure:

  • Place esophageal or rectal temperature probe

  • Place child on cooling blanket (per nursing guidelines)

 

Notification

  • Temperature < 35 º C or > 37.9 º C

  • Skin breakdown

  • Shivering
